• Carrier Packet
• Active Authority
• Signed W-9
• Proof of Insurance
• Copy of CDL
• $1,000,000 in Liability + $100,000 Cargo Insurance
• Automotive Liability – $1,000,000 Combined Single Limit
• New Entrants Passing Certificate (if less than 18 months in business)
• All insurance but Worker’s Compensation is to name Shipper as additional insured with primary coverage(only if shipper requires)
• CARB/TRU certificate (California)
• 30 Day Notice of cancellation
